Some of the predator fish near reefs such as barracuda, moray eel, parrotfish (coral eating), grouper and amberjack can be poisonous due to Ciguatera. Smaller reef fish which are consumed have low enough levels of this toxin to where it is not a hazard to humans. The larger predatory fish can become toxic due to the consumption of the smaller reef fish where the level of Ciguatera becomes elevated in their flesh. Being a former commercial fisherman and diver in the North Western Hawaiian Islands we always released any amberjacks we caught. I have consumed quite a bit of parrotfish and never suffered any ill effects. Back in the early 90's a couple of fisherman on the Big Island caught a barracuda while fishing from the shore and died from eating it. I've had grouper (which I shot myself) down in Belize and never became ill. Before moving back to the mainland I heard that the University of Hawaii had developed a test kit to check fish for Ciguatera toxin.
